The Northern Virginia Film Festival, a subsidiary of Rainmaker Media Foundation, is proud to host the 2009 Amateur Film Series. Winners will be determined on October 10, 2009.
Please print and complete this Submission Form and send it, along with three (3) copies of your Film Entry, the appropriate Film Entry fee (check or money order only, please), and any applicable releases to the Northern Virginia Film Festival, 12758 Lavender Keep Circle, Fairfax, VA 22033. ALL ENTRIES MUST BE SUBMITTED IN DVD FORMAT (NTSC REGION 0 OR 1). Please read carefully the Northern Virginia Film Festival "Rules and Submission Guidelines".

AFFIDAVIT OF ELIGIBILITY AND RELEASE
I certify that the information provided with this submission to the Northern Virginia Film Festival is true and complete. I have read the Northern Virginia Film Festival Rules and Submission Guidelines and I agree to abide by them. I am eligible to participate in the category to which I am submitting.
I grant permission to the Northern Virginia Film Festival (NVFF) and Rainmaker Media Foundation (RMF) to use clips of my film for promotional use.
I grant the non-exclusive right for NVFF and RMF to screen, exhibit and distribute my film in its entirety for any promotion or project under the festival or foundation's name for up to one year after the 2009 festival. NVFF and RMF may also actively pursue other distribution opportunities for my film.
I grant permission for NVFF and RMF and other third parties at their discretion the right to film, videotape and/or photograph my appearance or voice for any and all marketing and promotional activities for the festival.
My submission of this entry into the Northern Virginia Film Festival constitutes an agreement between me and NVFF and RMF. By submitting my film for consideration, I agree to all of these rules, terms and agreements.
I hereby voluntarily assume any and all risks, known or unknown, associated with my participation in the Festival and with my receipt and use of any Prize associated therewith. I and my heirs, executors, administrators and assigns hereby voluntarily forever release, discharge, waive and relinquish any and all actions, claims or causes of action against the NVFF, RMF, and their respective parent, subsidiary and affiliated and related companies, advertising and promotional agencies, administrators and prize suppliers, and their respective officers, directors, employees, agents and representatives (the "Released Parties") arising from or resulting in any manner from my participation in the Festival and/or my acceptance and/or use or misuse of the any Festival Prize.
